Job Description:

A rural, coastal town in Alaska is looking for an RN Diabetes Program Coordinator to be responsible for the implementation of the grant goals and objectives for the IHS Special Diabetes Program for Indians grant.   This person must be a Certified Diabetic Educator (CDE) or willing to obtain this certification within a year.   Incredible benefits, salary, and relocation assistance are provided!

 

Qualifications:

  • Must be an RN from any U.S. State.
  • Must be a Certified Diabetic Educator (CDE), or ELIGIBLE to obtain national CDE certification within one year of starting employment.
  • 3 years of experience working with diabetic programs preferred.

Primary Responsibilities:

  • Facilitates the development and communication of policies and procedures to employees within the Diabetes Program.
  • Organizes diabetic clinics and educational events, tracks diabetic patients, and maintains records for grant reporting.
  • Works collaboratively with the dietician, case managers, all care teams, and the Health Education Department.
  • Visits villages for education functions and organizes community outreach and health fairs in schools and community settings.
  • Provides nutrition and diabetes counseling to patients and their families.
  • Works collaboratively with the Grant Management Department to provide input regarding the needs of the program.
