Family Nurse Practitioner / Community Health Clinic

A community-focused healthcare organization in Central Pennsylvania, near Harrisburg, is seeking a full-time Family Nurse Practitioner to join its primary care team.

Job Responsibilities

In this role, you will:

  • Provide comprehensive primary care services to patients across the lifespan.
  • Conduct medical assessments, review patient histories, and develop individualized treatment plans.
  • Diagnose and treat acute and chronic conditions, including hypertension, diabetes, anxiety, depression, ADD/ADHD, and other behavioral health concerns.
  • Order and interpret appropriate laboratory and diagnostic testing.
  • Coordinate referrals and transitions of care between inpatient, emergency, specialty, and community-based settings.
  • Promote preventive care through screenings, immunizations, wellness visits, and patient education.
  • Educate patients and families on medications, treatment plans, disease management, and available community resources.
  • Collaborate with physicians, behavioral health providers, case managers, medical assistants, and other clinical team members.
  • Document patient encounters accurately and in a timely manner within the EMR.
  • Participate in quality improvement, value-based care, and team-based care initiatives.

Minimum Qualifications

  • Graduate of an accredited Nurse Practitioner program.
  • Current Pennsylvania Certified Registered Nurse Practitioner license.
  • Current Registered Nurse license in Pennsylvania.
  • Active Prescriptive Authority License.
  • Valid DEA Controlled Substance Registration, or ability to obtain prior to start date.
  • Successful completion of credentialing and privileging process upon hire.
  • Minimum of 24 months of experience as a Family Medicine provider.
  • Strong communication, documentation, and clinical decision-making skills.
  • Ability to work effectively with patients and staff from diverse cultural, ethnic, and socioeconomic backgrounds.

Preferred Qualifications

  • DNP preferred.
  • Experience in a community health, FQHC, or public health clinic setting.
  • Experience working with low-income, multi-ethnic, or underserved populations.
  • EMR experience.
  • Passion for health equity, whole-person care, and community-based medicine.
