Responsibilities of the RN Clinical Manager Care Continuum
- Responsible for coordination of clinical activities of the Population Health and Care Management Departments
- Role includes clinical practice, education, consultation, administration, research and professional leadership.
- Facilitate and coordinate planning of services to improve access, promote wellness, and improve health and safety.
Minimum Requirements of the RN Care Continuum Clinical Manager
- BSN is required
- 2+ years of population health/care management
- Registered Nurse in the state of California
