Hospital in Coastal Northern California is seeking a permanent Manager of Care Management. Master's degree in Nursing or Social Work with CCMC or ACM preferred. 5+ years of clinical experience with 3+ in case management along with 2+ year of experience in a supervisory role is required. Active license in the state of California.
Responsibilities of the Manager of Case Management
- Manages and assumes responsibility for the day to day operations of utilization management, care coordination and discharge planning.
- Responsible to purchase, educate and record education to new equipment and techniques.
