A full or part-time Licensed Independent Clinical Social Worker (LICSW) OR Licensed Clinical Mental Health Counselor (LCMHC) is needed in picturesque New Hampshire. Current LICSW or LCMHC with 3+ years of professional clinical experience. You will be responsible for evaluating, writing, monitoring, and coordinating behavior management plans with the clinical team in support of and consistent with direct patient care. This is in the clinical school-based setting.
Requirements:
· Current LICSW or LCMHC
· 3+ years of professional clinical experience
· Communicate effectively, make critical decisions, and demonstrate the knowledge and skills necessary to provide care.
Responsibilities:
· Works directly in treatment relationship with patients and families, individually or in groups
· Ensure service quality and participate in care coordination to ensure proper communication between clinical associates, patients, and referral sources
· Participate in performance improvement activities
· Maintain current practice knowledge
· Investigates case situations and presents information to Physicians and other members of the health team, on the patient’s family and social background pertinent to diagnosis and treatment
