Job Responsibilities of the Acute Care Social Worker
Performs professional-level social work in areas such as assessing patient needs and coordinating a safe, comprehensive discharge plan, which may involve emotional support to the patient, the family, or significant others.
Developing and implementing individual treatment plans; coordinating an intervention; solving problems; applying effective counseling techniques; establishing and maintaining effective working relationships with all personnel contacted in the course of duties; communicating with a wide variety of people from diverse socio-economic and ethnic backgrounds; efficiently, effectively, and safely using equipment; preparing written records and reports.
