Clinical Hospital Pharmacist

Our client located along the Columbia River in Washington State is seeking a Permanent Clinical Hospital Pharmacist to join their pharmacy team. Amazing dayshift schedule, no Sundays, alternating Saturdays up to 2pm. BS or PharmD.   Must have inpatient acute care hospital pharmacy experience.   2+ years of hospital pharmacy experience is preferred. Responsibilities of the Clinical Pharmacist:

• Pharmacist will demonstrate clinical competence as required by program needs

• Reviews, verifies, and processes provider medication orders ensuring appropriate therapy based on the patient’s
  condition and overall medical history.

• Provides direct patient care, including medication therapy management, patient counseling and therapeutic drug
  monitoring.

• Documents pharmaceutical care in patient’s medical record.

• Collaborates with healthcare teams to provide evidence-based medication recommendations.

• Participates in interdisciplinary rounds.

Requirements of the Clinical Pharmacist:

• Graduate of accredited School of Pharmacy. (Pharm.D. preferred)
• Must have Hospital Setting Inpatient Pharmacy experience
• 2+ years of experience in pharmacy
